It is important that all necessary documents are received 24-48 (business hours) prior to the date of the scheduled appointment. The office will contact you via phone, email, or text within 24-48 hours (business days) of your appointment to confirm that the required practice documents have been submitted. The documents include completed paperwork, insurance cards/information, identification, and a form of payment being held on file. Failure to provide the requested information may result in the cancellation of your appointment.
Insurance cards received on the day of the appointment will be verified the next business day. The scheduled appointment will be considered self-pay until verification is complete. You may request a more detailed description of our office policies from staff prior to scheduling your appointment.

Appointments & Cancellations
With the exception of serious emergencies, it is expected that you keep your appointments. If you need to reschedule an appointment, it is necessary that you advise the office at least 24 hours in advance. Your consideration in this matter will allow other patients to be accommodated who could benefit from an appointment.

Emergencies
Our office provides on-call service for emergencies. You must call so that the appropriate provider on-call can be contacted. After office hours, an answering service may take your message and immediately contact a provider with your information.
