The office will contact you via phone, email, or text within 24-48 hours (business days) of your appointment to confirm that the required practice documents have been submitted. The documents include completed paperwork, insurance cards/information, identification, and a form of payment being held on file. Failure to provide the requested information may result in the cancellation of your appointment.
Insurance cards received on the day of the appointment will be verified the next business day. The scheduled appointment will be considered self-pay until verification is complete. You may request a more detailed description of our office policies from staff prior to scheduling your appointment.
