You can request a prescription refill from your doctor online via the patient portal here. If you need a medication refill, you must be seen every 3 months. If a change in medications is needed prior to 3 months – please make an appointment with one of our providers.

Office Policies

It is important that all necessary documents are received 24-48 (business hours) prior to the date of the scheduled appointment. The office will contact you via phone, email, or text within 24-48 hours (business days) of your appointment to confirm that the required practice documents have been submitted. The documents include completed paperwork, insurance cards/information, identification, and a form of payment being held on file. Failure to provide the requested information may result in the cancellation of your appointment.

Insurance cards received on the day of the appointment will be verified the next business day. The scheduled appointment will be considered self-pay until verification is complete. You may request a more detailed description of our office policies from staff prior to scheduling your appointment.

Appointments & Cancellations

If you need to reschedule an appointment, it is necessary that you contact the office at least 24 hours (business days) in advance and speak with staff or leave a message. With the exception of serious emergencies, it is expected that you keep your appointments. Your consideration in this matter will allow for the accommodation of others who could benefit from an appointment. Please be aware that there is fee for same-day cancellations or no-shows.

Emergencies

Our office provides on-call service for non-life-threatening emergencies. After office hours, an answering service will take your message and immediately contact a provider with your information.
